Credit Application

Dear Potential Customer,
If you wish to apply for an account with North Star Coop, please download the Credit Application, print it out, and fill in the necessary information. We do charge a processing fee to process your application. This is a one time charge of $20.00 per application. If your credit application is approved, we will return your application fee to you. Once you have completed the application, please sign it, enclose your check or money order, and mail it back to us at:

Credit Policy

Please download our Credit Policy, read it, and retain it for your records. You do not need to mail a copy of the credit policy back with your application.
